2026-2027 CPC Student Information Form

We are excited to have you join us for the upcoming school year! 

Please complete and submit the following enrollment form as a student-parent team; both students and parents are asked to acknowledge and agree to the Annual Student Information Forms in the Student/Parent Handbook prior to submission. 

Important Note: This form must be submitted before the start of the school year for processing. Please submit this form by Friday, August 21.
